A surprising number of top-end hotels will also take dogs. Hiltons generally do, for example, but there is often a weight restriction (i.e. no dog over 15 pounds, or 25 pounds or 50 pounds). Restrictions and surcharges vary from hotel to hotel.

Hi end: Loews. The South Beach location is so swanky there's a page in the room service menu with food for pets, and dog-walking services are available.
My favorite "travel with dogs" hotels: La Quinta and Red Roof. Never a problem traveling with well-mannered big dogs with these chains. (Although the Red Roof in Tallahassee smelled as if it were marinated in harsh cleaning chemicals.)
Biggest problem at the low end: Because you're "high-risk," they'll often try to force you and your dogs to accept smoking rooms, figuring those are already ruined. Ask. If that's their policy, go elsewhere.
my experience with almost all of the chains you list is that some individual hotels DONT accept dogs, or have weight/number or even breed limitations. The only chain I've found that uniformly accepts dogs is Motel6 (though it's pretty low end for a lot of people). Even though the signs at the check in say "one dog", I've never been denied staying with 2-3
